You say "Vado al bagno" (I'm going to the Bath) when you're going to a room with a shower in order to take a bath. All other places where you can clean up your hands or pay heed to nature's call are called "toilette".
As an aside, you may have noticed that the word toilette does not look very Italian. That's because it's a loan word from French.

Wow! I never thought about that and it did cross my mind the first time I saw "toilette" that it didn't seem very italiano!!
