"Que quieren ustedes?" translates to "what would you like?". So most likely in a real life situation the person that you were speaking with would understand that you were referring to food, given the right context, even though you did not directly state it. Does that help?

It helps a little. I think my question is whether or not the construction of the sentence changes because of "que" being at the beginning. If I said "do you want" it would be "ustedes quieren," but "what would you like" is "que quieren ustedes." I'm wondering if that is always the case. It might just be a situation where I need to know to just put quieren first.
