"v" has a very slight "z" like pronunciation, "b" does not. "b" is the pure "b" pronunciation and "v" is mixed with a slight "z" pronunciation. Spain is the king of using this type of pronunciation, in latin america is rarely use and t obe honest, "b" and "v" are pronounce almost if not identically the same.

Yes, I have noticed native speakers usually use a "b" sound for many of the "v" sounds. But it doesn't seem arbitrary, if they use a sound on a specific word, they're consistent. I've kind of adapted by closing my lips as if making a b, but then making the v sounds for v's, and using b for b words like baño.
What I've noticed is that the DVD's pronounce "yo" with a j sound (like "jo" quiero), whereas some of the native speakers I hear say it like a y sound. Same with the ll, sometimes a hard j sound like llamar and sometimes a y sound like se llama. I think that's part of my difficulty listening since I seem to visualize the words as I translate them in my head - so since people use different soundings, you have to be aware of it.

Yes, you are absolutely right, I managed by assuming it 'v' with little voice of 'b'...
I am not sure if this would helped, but i wuonder why it was not very clear in the DVD lessons. I hope they just talked about it. May be it's in CD audios but have not checked it yet.
