The structures are certainly not the same, and some are preferred more than others. But all of them work and mean the same thing. However, you usually want to put the adjective after the noun in Spanish, especially if you use the word "muy". In this case, the adjective being "bien", and the noun being "español". Thus, the last one, "usted habla español muy bien", is the best choice of the three.

Actually, isn't "bien" an adverb? I would think the first option is preferable, but I am not a native speaker.
