on second thought, maybe that should be reflexive: Gli ho fatto un'offerta che non si è potuto rifiutare. Is he refusing the offer to himself, refusing the offer, or refusing to cooperate with the Godfather? Maybe it works both ways.

I don't know if it would be reflexive or not….he would be refusing the offer (or couldn't, in this case), but I don't see how that action would "reflect" back on him. Then again, reflexives give me grief!

I would say it's not reflexive. You can't "offer" yourself, at least in the way suggested here.

After thinking it over I’d say I was right the first time.
The Godfather (I), is the subject of the sentence. “Him”, is the direct object, so the “not refusing” is an action directed at the Godfather, not back on himself.
Oh well, as they say:
