I agree, comfort comes with experience. Each skill supports the others, but if you want to read better, you have to read. If you want to speak better, you have to speak. No matter how much you've studied, your first few times will feel awkward and clumsy, but they'll work with you, and you'll improve.
One thing you might want to do is join a foreign language exchange website. These services connect you with another student who's learning your language, so for example you can help with their English and they'll help with your Spanish. I won't name names, but there are several out there, depending on how you want to communicate (posting a journal, finding pen pals, instant messaging, Skype, that sort of thing.) I've never had a problem with native speakers getting impatient or criticizing my accent, but in a language exchange, the person on the other end of the conversation is also a student, so you may feel like you're on a more equal footing.
